Terms and Conditions

1. To enjoy the offers under this “Healthy ‘Tea-Check’ Offer” (this “Offer”), each participant (“Participant”) must make an appointment registration for each selected programme that he/she wishes to participate within the period commencing on 1 May 2025 and ending on 31 October 2025 (both days inclusive) (“Registration Period”) in accordance with clause 4 below, and complete the first doctor consultation of each programme registered on or before 31 December 2025.
2. Each Participant is deemed to have read, accepted, and agreed to be bound by these terms and conditions upon making an appointment registration for any service under this Offer.
3. This Offer is offered on a first-come, first-served basis, subject to the service availability at CUHK Medical Centre or CUHK Medical Clinic (Tsim Sha Tsui) (as the case may be).
4. Appointment Registration:  
   a.    For selected programme(s) provided at CUHK Medical Centre – Participant must register an advance appointment for the relevant selected programme(s) during the Registration Period by submitting an online appointment enquiry form for booking (https://bit.ly/CUHKMC_HEALTHY-TEA-CHECK-OFFER_WEB-FORM) or by calling 3946 6188. A booking is considered confirmed after a confirmation message by SMS/WhatsApp is sent by CUHK Medical Centre to the Participant. 
   b.    For selected programme(s) provided at CUHK Medical Clinic (Tsim Sha Tsui) – Participant must register an advance appointment for the relevant selected programme(s) during the Registration Period by calling CUHK Medical Clinic (Tsim Sha Tsui) hotline at 3468 7141.
5. The services provided under this Offer are only available to persons aged 18 or above.
6. Each Participant has to agree to and accept the relevant service terms and conditions of CUHK Medical Centre or CUHK Medical Clinic (as the case may be) before receiving the services provided under this Offer.
7. The relevant registered medical practitioner(s) of CUHK Medical Centre or CUHK Medical Clinic (as the case may be) has the sole discretion to determine a Participant’s suitability to receive the services under the selected programme(s) that he/she has registered for. If such medical practitioner determines that the Participant is not suitable to receive such services, CUHK Medical Centre or CUHK Medical Clinic (as the case may be) will only charge the Participant doctor’s consultation fee and (if applicable) any related diagnostic and/or medication fees/charges according to its/their prevailing price(s).
8. This Offer cannot be used in conjunction with other offers or discounts. 
9. This Offer is not transferrable and cannot be exchanged for cash or other products or services. Any fees paid for the services under this Offer are not refundable and cannot be exchanged for other products or services.
10. CUHK Medical Centre Limited and CUHK Medical Clinic Limited reserve the right to amend any provision of these terms and conditions at any time without prior notice. 
11. In case of any dispute, the decision of CUHK Medical Centre Limited and/or CUHK Medical Clinic Limited, as the case may be, shall be final and conclusive.
12. These terms and conditions shall be governed by, and construed in accordance with, the laws of the Hong Kong Special Administrative Region of the People’s Republic of China.
13. No person other than the Participants and CUHK Medical Centre Limited and/or CUHK Medical Clinic Limited will have any right under the Contracts (Rights of Third Parties) Ordinance (Chapter 623 of the Laws of Hong Kong) to enforce any provision of these terms and conditions.
14. In the event of inconsistency between the English and the Chinese versions of these terms and conditions, the English version shall prevail.
